Upper and lower canines need separate decisions
For an upper canine displaced towards the roof of the mouth, options may include monitoring, carefully selected early removal of the baby canine, surgical exposure followed by orthodontic alignment, or removal of the permanent canine if alignment is not practical. These choices depend on the canine's position, the health of nearby roots, available space, age and willingness to have orthodontic treatment. Evidence does not establish that early removal of a baby canine will prevent the need for later treatment; it is only considered in selected cases.
Lower impacted canines have a different and thinner evidence base. Published reports describe monitoring, removal, or exposure followed by orthodontic movement, but do not establish one preferred option or a predictable success rate. The upper-canine guidance should not be applied automatically to a lower tooth.

What assessment can establish
The pattern of teeth coming through the gum and the examination, including feeling for the canine under the gum, can help identify displacement. Dental X-rays may be used when clinically indicated to locate the tooth and assess nearby roots. A concern about an upper canine can matter because an unfavourable position may damage the root of an adjacent front tooth (incisor), but that is a possible risk—not something to assume without examination and appropriate images.
If exposure and orthodontic alignment are chosen
Surgical exposure uncovers the canine's crown, the upper part of the tooth normally above the gum. In some upper-canine plans, a small bracket and chain may be attached, or a plate or pack may protect the area. The orthodontic team then guides the tooth into position over later visits. The exact exposure method and protective dressing depend on position and the joint plan; uncovering the tooth does not move it immediately or guarantee alignment. This is a coordinated surgical and orthodontic pathway, not a claim that one clinician provides both specialties.
For an upper canine far out of position, bringing it into alignment may be impractical. In adults, alignment can take longer or be less predictable. Removing an upper canine that lies toward the roof of the mouth may injure neighbouring tooth roots. These are possibilities to discuss for the individual position, not predictions for every case, and evidence does not establish one exposure technique as universally preferable.
Care after exposure
Follow the surgical team's mouth-care instructions and protect any chain, plate or pack from being pulled or knocked loose. Attend the arranged surgical check and orthodontic reviews. Contact the team if a pack comes away or if a chain or plate causes a problem; the instructions depend on the selected technique. Heavy bleeding or severe pain after oral surgery also warrants contact with the surgical team.
