Reassess the cause and the site
The clinician reviews previous treatment, the implant and restoration, surrounding tissues and available records. Medical conditions and medicines should be considered before implant treatment. No symptom alone establishes the cause of failure or whether a replacement can be supported. Pain or perceived implant looseness should be reported promptly for dental assessment; these signs do not diagnose failure.

Replacement is one option among several
A new implant can sometimes be placed where an earlier implant failed, but reviewed evidence suggests the chance of success is lower than for first placement.
